Summary Job Description: Physical Therapist Full Time, First Shift The Physical Therapist I shall evaluate and treat patients; develop a treatment plan; and set goals for patients. Patient and family education is included in the treatment plan and goals. Job

Duties

Completes appropriate functional evaluation including assessment of pain for various musculoskeletal/neurological conditions. Plans treatment appropriately including input from patient and/or caregivers. Provides services to improve functional independence and/or reduce pain such as: therapeutic procedures/exercises or activities to promote ROM, strength, coordination, balance, functional mobility (bed mobility, transfers, floor transfers), ADL training, gait training, balance and vestibular training, education, and use of assistive devices. Receives and implements therapy orders. Completes evaluation form, treatment notes, progress reports and D/C summaries in a timely manner and appropriately submits to referring provider.

Requirements

Bachelors in Physical Therapy Eligible for a temporary license or Licensed as a Physical Therapist by the Kentucky Board of Physical Therapy Preferred
