Estimated Appointment End Date (Continuous if Blank) Full/Part-Time Full time Job Type Regular Compensation $33.00 USD Hourly Job Description Basic Purpose Positions in this job family are assigned responsibilities for performing technical direct and indirect nursing care in a clinic, hospital, institution, or home setting for individuals under the supervision or direction of a registered nurse, licensed physician, or dentist. This includes taking temperatures, pulse, respiration, and blood pressure, and various other technical nursing tasks in providing health care to patients and others. Typical Functions The functions performed in this job family will vary by level, unit and organization, but may include the following: Perform nursing care within the Oklahoma LPN-IV scope of practice, including medication administration, wound treatments, resident assessments, documentation, and resident education. Support implementation of individualized care plans while promoting evidence-based wound management and infection prevention practices. Maintain competency in wound care, infection prevention, and regulatory requirements through ongoing education and professional development. Promote resident safety, skin integrity, and pressure injury prevention through collaboration with the interdisciplinary healthcare team. Perform additional clinical and administrative duties as assigned to support resident care and departmental operations. Serve as the primary clinical support to the Wound Care Coordinator by assisting with the facility's wound care program. Participate in scheduled wound rounds with the Wound Care Coordinator, Consultants and Providers to assess wound progression and evaluate treatment effectiveness. Review daily reports, provider orders, and nursing documentation to identify residents requiring wound assessments, follow-up, or treatment changes. Assist with prioritizing wound care services to ensure timely assessments, treatments, and provider follow-up. Collaborate with nursing staff to ensure wound care interventions are completed according to physician orders and facility protocols. Perform wound observations and treatments within the Oklahoma LPN-IV scope of practice and under the direction of the Wound Care Coordinator and Provider. Complete scheduled and as-needed wound treatments utilizing evidence-based wound care practices. Assist the RN with wound measurements, staging, photography, documentation, and monitoring for changes in healing status. Maintain accurate wound documentation within the electronic medical record. Ensure treatment supplies are available, and wound care equipment is maintained according to infection prevention standards. Collaborate with the Wound Care Coordinator to communicate changes in wound status to the Provider promptly. Assist with updating wound care orders in the electronic medical record following provider recommendations. Collaborate with the Provider LPN IV for follow-up appointments with wound specialists, surgeons, or other consultants as directed. Communicate changes in treatment plans to nursing staff and ensure new orders are implemented timely. Participate in interdisciplinary care planning to promote optimal wound healing and prevent complications. Collaborate closely with the Infection Preventionist LPN IV and Wound Care Coordinator to identify wounds. Monitor wounds for signs and symptoms of infection and immediately report concerns to the Provider and Wound Care Coordinator. Collaborate with the Lab/Diagnostic LPN IV, Provider LPN IV, Wound Care Coordinator, and medical provider. Ensure timely implementation of provider orders related to infected wounds and monitor resident response to treatment. Serve as the primary backup for the Infection Preventionist LPN IV during planned or unplanned absences. Serve as the primary backup for the Lab/Diagnostic LPN IV. Collaborate with interdisciplinary team members to maintain continuity of resident care across specialty nursing services. Maintain accurate wound care documentation in accordance with facility policies, CMS regulations, and professional nursing standards. Assist with wound tracking logs, quality indicators, prevalence and incidence reporting, and regulatory audits. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives related to wound prevention, treatment outcomes, and pressure injury reduction. Level Descriptor Levels are distinguished based on the complexity of assigned duties, the level of expertise required for completion of work assignments, and the responsibility assigned for the supervision of others. This is the leadership level of the job family where employees are responsible for providing supervision to Licensed Practical Nurses (LPN) in the care and treatment of patients. This also includes supervising Patient Care Assistants (PCA) serving as Certified Nurse Aides (CNA) and Certified Medication Aides (CMA).

requirements at this level consist of possession of a valid permanent Oklahoma license as a licensed practical nurse as approved by the Oklahoma Board of Nursing plus three years of practical nursing experience; or Licensed Practical Nurse License recognized by the Enhanced Nurse Licensure Compact (eNLC) plus three years of practical nursing experience. Requirements

Some positions may require that applicants be willing and able to fulfill all job-related travel normally associated with the position. Upon a conditional offer of employment, Department of Mental Health and Substance Abuse Services applicants must submit to a physical examination and shall meet the physical examination requirements of occasionally lifting/carrying 50 pounds and able to push and pull a maximum force of 25 pounds. Applicants with the Department of Corrections must be willing and able to work any assigned shift or day of the week, and additional shifts as required.
