$28.55 - $42.85 Help Patients Navigate Care and Improve Lives Atrium Health Wake Forest Baptist is seeking a compassionate Patient Navigator - Human Services to support patients through complex healthcare journeys. This role serves as a bridge between patients, healthcare teams, and community resources, helping individuals overcome barriers to care, stay engaged in treatment, and access the support they need to achieve better health outcomes. Candidates with experience in HIV care, Ryan White programs, infectious disease clinics, patient advocacy, or community-based health organizations are strongly preferred. Key

Responsibilities

Connect patients to healthcare, social service, and community resources. Coordinate care alongside providers, case managers, and social workers. Identify and address barriers to treatment, including transportation, medication access, stigma, and financial challenges. Conduct patient outreach, appointment reminders, and rescheduling. Analyze no-show trends and help re-engage patients in care. Document patient interactions accurately and timely. Coordinate peer advocacy and volunteer support programs. Serve as a liaison between patients, families, healthcare teams, and community organizations. Required

Qualifications

Associate's degree in Social Work, Sociology, Psychology, Child Development, Nursing, or related field. Two years of experience in healthcare, human services, case management, patient advocacy, community outreach, or a related setting.

Preferred Qualifications

Bachelor's degree in a related field.

Experience

working with HIV/AIDS populations or within a Ryan White-funded organization or clinic.

Experience
