Work Shift

Day Work Days: MON-FRI Scheduled Hours: 7 AM-3:30 PM Scheduled Daily Hours: 8 HOURS

Scheduled Weekly Hours

40 Hourly Rate/Range: Starting at $36.00 For positions that have only a rate listed, the displayed rate is the hiring rate but could be subject to change based on shift differential, experience, education or other relevant factors.

Job Summary

The candidate should have experience with the patient assessment, exercise prescription, care plan, ECG telemetry monitoring, patient education patient scheduling and supervision of cardiac patients. Excellent communication and social skills are essential.

Essential Functions

1. Conduct new patient tours and interviews. 2. Completes patient scheduling to manage daily workflow. 3. Prepare and upgrade initial exercise prescriptions. 4. Conduct new patient orientation 5. Supervision of Phase 2 exercise classes. 6. Prepare summary reports for Phase 2 patients at the end of their 10th week of program participation. 7. Monitor graduation dates and conduct graduation consultations at 30th session. 8. In consultation with Program Director, prepare medical necessity reports in response to insurance carrier requirements. 9. Prepare medical alert documentation for referring physicians. 10. On a regular basis, check for and reorder necessary exercise program supplies. 11. Maintain daily quality control for all exercise telemetry equipment. 12. Maintain adequate qualities for all program forms. 13. Prepare maintenance exercise program for new Phase 2 graduates. 14. Prepare home exercise programs with associated approval of primary M.D. 15. Daily check all emergency medical equipment and crash cart. 16. Be familiar with all Hospital Code procedures and protocols. 17. Participate in weekly summary conference meetings with program director. 18. Provide as needed ECG monitoring and documentation of exercise classes. 19. Conduct patient education lectures. 20. Perform other duties as required by the Cardiac Program Manager.

Qualifications

Bachelor Degree Required or Masters Preferred 4-6 years

Experience

in Excercise Physiology Preferred ACLS Advanced Cardiac Life Support - American Heart Association Required BLS Basic Life Support - American Heart Association Required ACSM - Certified Exercise Physiologist - American College of Sports Medicine within 1 Year Required CCRP Preferred AACVRP Preferred Montefiore Nyack Hospital is an equal employment opportunity employer.
