Job Description: The Behavioral Health Care Manager plays a key role in delivering integrated mental health services within the Collaborative Care Program. This position is responsible for conducting mental health screenings and assessments, as well as providing evidence-based treatments for conditions such as depression, anxiety, and traumatic stress in pediatric patients with chronic illness. The Behavioral Health Care Manager educates patients on managing their conditions, monitors clinical outcomes, and tracks symptom progression or complications. Utilizing therapeutic approaches such as Cognitive Behavioral Therapy (CBT) and Motivational Interviewing, the Behavioral Health Care Manager delivers brief, targeted interventions. This role works closely with nephrology providers, psychiatric consultants, and caregivers to develop and implement individualized treatment plans. Additional responsibilities include care coordination, advocacy, case reviews, and facilitating transitions across levels of care. This position reports directly to the pediatric Med Psych Program, with a connection to the Proactive Care Support Services. Job Specifics Application

Essential Functions

Performs screening, assessment, and treatment of depression, anxiety, and traumatic stress, providing patient education around chronic disease management and available treatment options. Tracks and documents patient follow-up and clinical outcomes, monitoring patients for changes in clinical symptoms, treatment side effects, or other complications. Provides brief therapeutic interventions to patients using evidence-based techniques such as Behavioral Activation (BA), FAST-A, FAST-D, Cognitive Behavioral Therapy (CBT), Motivational Interviewing, and Acceptance and Commitment Therapy (ACT). Consults with the nephrology providers, psychiatric consultant, and patient/caregivers to facilitate care plans, inclusive of medication management, psychosocial treatments, or referrals for additional services. Acts as an advocate on behalf of patients, where appropriate, ensuring effective communication with the multidisciplinary care team, outside agencies, and other resources. Works with the multidisciplinary team to develop a discharge plan to provide further care, services, and referrals in order to ensure continuity of care. Maintains up-to-date knowledge of best practices and emerging trends in behavioral health care. Effectively assesses and manages suicidal and homicidal risk, as well as concerns for child abuse and neglect, using effective and evidence-based methods. When applicable, collaborates and coordinates care with the Crisis team, contacts police and/or Child Protective Services, and communicates effectively with the receiving emergency department. Qualifications

Current licensure in the state of practice as a Licensed Clinical Social Worker (LCSW), Clinical Mental Health Counselor (CMHC), or Marriage and Family Therapist (MFT). Current licensure in LLMSW, CSW, ACMHC, or AMFT in state of practice acceptable but must obtain LCSW, CMHC, or MFT within one (1) year of hire or promotion into this role. Master’s degree in Social Work, Psychology, Counseling, or a related field from an accredited institution (degree verification required). Demonstrated experience in behavioral health care, preferably in a collaborative care setting. Strong understanding of evidence-based therapeutic techniques and interventions. Excellent verbal and written communication skills. Ability to work effectively within a multidisciplinary team. Proficiency in electronic health records (EHR), documentation, and Microsoft 365 products. Commitment to patient advocacy and providing patient-centered care.

Preferred Qualifications

Current licensure in the state of practice as a Licensed Clinical Social Worker (LCSW), Clinical Mental Health Counselor (CMHC), or Marriage and Family Therapist (MFT).

Experience

and expertise in clinical practice within an ambulatory care setting, community health, or home care.

Experience

working successfully in a remote environment, including proficiency with advanced Microsoft Suite tools such as Teams (chat, whiteboard, task tracking) and Microsoft Outlook.

Experience

in care management, care navigation, or a closely related field such as discharge planning, managed care, behavioral health, or patient education. Ability to work independently, demonstrate self-motivation, maintain a positive attitude, and adapt to a rapidly changing environment. Strong written and verbal communication skills.

Physical Requirements

Ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and be able to assess customer needs. Frequent interactions with customers that require employee to communicate as well as understand spoken information, alarms, needs, and issues quickly and accurately. Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer, phone, and cable set-up and use. Expected to lift and utilize full range of movement to transport, pull, and push equipment. Will also work on hands and knees and bend to set-up, troubleshoot, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items. For roles requiring driving: Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
