Pay Range

$51.05 - $76.60 Provide inpatient palliative care consultations and follow-up care for patients with serious illness across the AHN Palliative Care Program. Manage initial and follow-up palliative care consultations, prioritizing patients by acuity and collaborating with a dynamic Interdisciplinary team. Support complex symptom management, serious illness communication, goals-of-care discussions, hospice evaluations, and end-of-life care planning. Location & Setting Primary Program AHN Palliative Care Program Care Setting Inpatient palliative care consult service Employment Status Full-time Scope of Practice / What You’ll Do Provide initial and follow-up palliative care consultations for inpatient patients. Partner with physicians for collaboration while functioning at the top of the APP scope of practice. Support patients and families through symptom management, hospice evaluations, goals-of-care conversations, and end-of-life planning. Participate in interdisciplinary collaboration with physicians, APPs, hospice liaison support and palliative counselors. Procedures are not required for this position. Qualifications

Nurse Practitioner with appropriate Georgia licensure required or ability to obtain prior to start. Acute Care NP, Primary Care NP certification required, as applicable to licensure and role scope. New graduates will be considered. 2 to 3 years of Hospice and/or Palliative Care experience, or related experience caring for patients with serious illness. Hospice and Palliative Medicine (HPM) certification preferred. Strong skills in symptom management, serious illness communication and end-of-life care.
