Position Highlights We are seeking a dedicated Nurse Practitioner or Physician Assistant to join our expanding Transitional Care Management Team. This role focuses on caring for high-risk patients recently discharged from the hospital, with the goal of improving outcomes, reducing readmissions, and ensuring a smooth transition back to the community.
Responsibilities
Provide Transitional Care Management services to recently discharged patients. Collaborate closely with hospitalists, Emergency Department providers, and Case Management teams at Atrium Health Lake Norman Hospital. Work as part of a multidisciplinary team including Registered Nurses, LPNs, Pharmacists, and Social Workers. Office hours are Monday through Friday.
