Job Summary
The Clinical Pharmacy Specialist – Collaborative Drug Therapy Management (CDTM) provides advanced clinical pharmacy services across designated ambulatory clinics under a physician-executed written agreement and in full compliance with New York State

Law Article 137 §6801-a and Part 63.10. The specialist manages and adjusts medication therapy, orders and evaluates appropriate laboratory tests, generates new prescriptions consistent with approved protocols, and documents all care activities in the electronic health record. This role works collaboratively with physicians and other members of the care team to support safe, evidence-based management of chronic conditions, contributes to the development and continuous improvement of CDTM activities, provides education to patients and clinical staff, and ensures accurate and compliant billing for clinical pharmacy services, while functioning independently within an authorized scope of practice.
Essential Functions
The Clinical Pharmacy Specialist-CDTM participates in all, but not limited to, of the following activities: 1. Demonstrates and applies knowledge of evidence-based medication management for chronic and complex conditions as necessary for the designated ambulatory care clinic in order to optimize therapeutic outcomes 2. Law Article 137 §6801-a, Part 63.10 regulations, institutional policies, and CDTM agreements while working collaboratively with physicians and interdisciplinary team members to coordinate care and implement authorized drug therapy adjustments. 3. Provides timely, accurate, and compliant documentation of clinical encounters, laboratory orders, and prescription management, and ensures proper billing for CDTM and ambulatory pharmacy services. 4. Communicates medication information clearly, supports patient understanding and adherence, and involves patients in shared therapeutic decision-making within the CDTM framework. 5. Participates in evaluating clinical outcomes, identifying opportunities for improving CDTM services, and contributing to protocol development, performance metrics, and continuous quality initiatives. * Document on addendum age specific competency. Provides escort to customers when necessary. PERFORMANCE IMPROVEMENT: 1. Participates in Department/Hospital-wide activities which improve processes and/or outcomes. 2. Utilizes resources to maintain and improve self development and knowledge. 3. Complies with licensure and/or certification requirements. 4. Participates in annual competency assessment. BUDGET: 1. Demonstrates fiscally prudent use of equipment and supplies. 2. Utilizes charge process for supplies/equipment. PROJECTS/ASSIGNMENTS: 1. Completes projects/assignments within projected time frames and expectations.
Qualifications
PharmD Required Residency PGY-1 Required PGY-2 training in ambulatory care or additional 2-3 years of clinical ambulatory care experience Required 2-3 years ambulatory care clinical experience in a hospital-based and/or CDTM-modeled setting is required.
